At Lakeside Pain Center we are committed to providing the highest-quality, research-supported, patient-centered pain management care. In an effort to provide high quality care we first would ask the opportunity to review your previous medical history, including:
– Previous treatment records including CT scans, MRIs, injections, and surgeries related to your pain.
– Previous documentation from another pain provider, if you have ever been treated by one.
We ask that you contact your primary care provider and have them send the medical records. Our fax number is 605-753-0017.
After we have had a chance to review this we will contact you for a new patient appointment.
